Payments

 

It is the responsibility of the patients to be aware of and understand their health and vision insurance coverage. In an effort to keep cost down our practice does not bill for services rendered, payment for any co-insurance, deductible or co-payments will be collected at the time of services. Should special arrangements need to be made please contact the Office Manager prior to your visit to make arrangements.

Our practice offers a discount to services that are not covered and for un-insured persons. Discounts do not apply to any service covered by insurance.

Refractions are a necessary component for many exams, to determine a patients eyeglasses or contact lens prescription, following medical problems with the eyes, prior to and/or following surgical/laser procedures and for patients taking some medications. Some insurance companies do not cover this service, the fee is $15.00. Medicare, Choices Plus, Humana, Humana Gold Plus and Medicaid are a few that DO NOT cover the refraction fee.

For your convenience we accept the follow methods of payment: Cash, Checks, Master Card, Visa, Discover, American Express and Debit.

 

 

Have you insurance cards with you, our staff will verify coverage at every visit.

Monday - Thursday 8:00 AM until 5:00 PM
Friday 8:00 AM until 5:00 PM

New Patients:
Please arrive 15 - 20 minutes prior to your scheduled appointment time to complete paper work. Have with you a state issued ID or driver license and all insurance cards.

Please bring with you a current list of all medications you are presently taking, with the correct name of the medication, strength and dosage.
